The important immediate task for a TPM is to identify the blockers and work
towards unblocking the same.
The reason for delay could be due to multiple reasons,
1.  Lack of information - due to no proper handover, no proper documentation.
However, we can't spend more time on documentation at this point of time. Just
take a crips pause, bring up a confluence page or central repository with clear
high level details of 
a. what is the deliverable of the project
b. what has been completed already
c. upcoming delivery what is pending from each cross functional teams.

2. More bugs in the software 
a.. If we can get more testing done - incorporate testers if possible. 
b. More frequent and focused delivery.
c. If we can collaborate with other teams /customer for testing and early bug
detection we must go for it.
3. communication across teams is causing the delay 
a. Bring weekly calls or bi weekly calls in place. If needed facilitate the
daily calls which are focused on only unblocking the dependency.
b. If the budget permits go for on roof integration till the situation
improves.

4. 3rd party dependency 
a. A Project plan highlighting the dependency from 3rd party must be prepared
and published with the stakeholders. Any influence on unblocking 3rd party to be
made.
