Not enough planning. Sometimes programs are created in a rush without adding enough detail to the execution plan. This problem usually causes problems in communication and resource underutilization as tasks and dependencies are being planned along the way.
No risk management. Sometimes do not manage risks proactively or do not document resolutions or mitigations. This problem usually causes slow downs in program execution as problems are addressed too late in the process when they become blockers.
Poor communication. Sometimes the communication plan is not documented as part of the program plan and communication is not consistent or frequently enough. This problem causes misalignment between stakeholders and the teams participating in the program.
Low trust among program participants. This usually causes poor communication, misalignment and poor execution.
Haven’t seen all these issues in different programs throughout my career, I have learned to identify them and prevent them. Starting with detailed planning, continuous risk management, making communication part of the execution plan and adapting it as the program progresses, fostering open and honest communication to ensure trust between program participants is strong.
